VBS Volunteer Information

We are always in need of adult, high school-aged and even middle school-aged (entering 7th and up) volunteers for VBS! All volunteers aged 18 and older must be Virtus-trained. Students may use service hours for any club/school/sacrament prep requirements: Minimum of 20 hours! Sign up below to help us bring our young people closer to Christ.

Volunteer <18 years old: Minor Participation Consent

I, as the parent/legal guardian of the above-named child, agree to the Permission/Waiver below.

I, as the parent/legal guardian of the above-named child, do hereby grant permission for my child to participate in the St. John Neumann Vacation Bible School on July 20-24, 2026 and therefore agree by my signature to release, absolve, indemnify and hold harmless St. John Neumann Church, the Roman Catholic Diocese of Cleveland, the Bishop of the Roman Catholic Diocese of Cleveland, and any and all supervisors, employees, organizers, or sponsors of and from any and all liability for any injury, medical fees, hospital bills, or doctor bills for my aforesaid child.

Consent for Emergency Treatment*

In case of emergency, (and in the event reasonable attempts to contact a parent/legal guardian have been unsuccessful, if applicable,) I give my permission to provide any medical treatment, care, or attention that is required. This authorization does not cover major surgery, unless the medical opinions of two licensed physicians or dentists, concurring on the necessity for such surgery, are obtained before surgery is performed.

I hereby give St. John Neumann Parish permission to publish photographs taken of me/my child, for use in St. John Neumann printed publications, in-house displays, and on the parish social media and web site. I release St. John Neumann from any expectation of confidentiality for myself/my child, and, as applicable, I attest that I am the parent or legal guardian of the child on this registration form, and that I have the authority to authorize St. John Neumann Parish to use their photographs.
